Rule out fibroids and other cancerous growths early



Fibroids and cancer are very common nowadays, especially between 30 -40 years of age group. While Cancer is most prevalent post Menopause. Occurrence of Fibroids is predominantly seen in 80% of the woman when they are reaching 50.

The solution to this is undergoing D&C procedure. D & C is through biopsy of uterine tissues one can screen the prevalence of cancers at early stage.

What is D&C
Dilatation and curettage is gynaecological procedure wherein the cervix (opening) of uterus is dilated, and the lining of uterus (endometrium) is removed with an instrument called curette.
 Why D&C is done
  • To find the cause of abnormal bleeding from uterus
  • Helps in diagnosis of cancerous cells by doing biopsy after taking tissue
  • After miscarriage to remove the retained products of conception
  • Helps in removing polyp tissue overgrowths
Some contraindications
  •  Suspected pregnancy
  •  In cases of arthritis of hip where there is difficulty in keeping legs apart
  • Patient is physically weak and ill
Pre-operative

Patient should not take food and water 7 hours prior to the procedure.

Anaesthesia

  • General anaesthesia is given and patient can go home on the same day
Procedure for D&C
  • The cervix and vagina are cleansed with betadine (antibacterial scrub) after administration of anaesthesia to the patient.
  • The upper part of cervix is grasped by an instrument and then metal dilators are used to widen the opening of uterus up to the size of a large pencil.
  • After the process of dilation, an instrument having a flat metal loop at its end , is used to scrape the uterus lining after inserting it in the cavity of uterus
  • The surgeon stops scraping when he feels the grittiness of layer of cells just above uterine muscle.
  • The whole of inside of uterus is scraped and the obtained tissue is sent for microscopic pathological examination at laboratory
Complications of D & C
  • Patient experiences abdominal cramps, vaginal bleeding.
  • Narrow cervical opening or abnormal internal anatomy of uterus may lead to perforation
  • In some cases tears or cuts in the uterus.
  • Infection in uterus
Management
  •  Antibiotics are prescribed to avoid infections
  •  Tab Methergin is given for controlling bleeding.
  •   Tab Paracetamol 500 mg is given to avoid fever.
